Our Mission
Our mission is simple safety, supportive, and person-centered environments where individuals can thrive not just survive. We work in close partnership with each person, their families, local authorities, and healthcare professionals to develop flexible support plans that promote independence, choice, and inclusion in the community.
With a team of compassionate, well-trained staff, we focus on building meaningful relationships, daily living skills, and confidence enabling each person to lead a fulfilling life on their own terms. Whether it’s support with everyday activities, community engagement, or personal development, we are here every step of the way.
At Total Care, we don’t just provide support, we create opportunities for people to live the lives they choose.

Supporting Individual
Our services support people with a range of needs. We provide a level of care and support which is appropriate to the needs of the individual using positive behavioural support, active support and person-centred planning.
We support individuals to live active, healthy, socially responsible and socially inclusive lives as part of their local communities. We also engage with a wide range of community organisations and emerging natural support networks. Keeping individuals safe in the community is important to us so we proactively identifyrisks and develop strategies to ensure this.
Communication difficulties are common amongst people with learning disabilities and can present barriers to accessing community services, social activities and health care. We ensure that our staff are trained to effectively communicate and advocate on behalf of the individual being supported.

Residential Care
We provide care and support for adults with learning disabilities, autismland behavior’s of concern. Our person-centered planning ensures that every person living in our residential homes is enabled to meet their physical, emotional, vocational, educational, spiritual, safety and social needs, 24 hours a day.
More than just a place to live – we want to help make homes. Our team ensure individuals are included in all decisions about their care and support, either directly or through families or advocates. This could include choices about menus, activities, or holiday destinations.
We offer opportunities for individuals to learn new skills, become more independent and try new experiences. As well as providing a full itinerary of in-house activities, such as craft sessions, music therapy, intensive interaction, sensory sessions and support to complete their pathway plan we also encourage residents to become active members of the community.
This can include using local shops and cafes on a regular basis, looking for volunteering opportunities, employment and attending courses at local colleges and libraries. Each of our homes have been carefully designed and adapted to meet the needs of every person who lives there. All our rooms have an on-suite, and most properties have a large multi-function sensory room.
Positive Behavior Support
We use Positive Behaviour Support (PBS) to reduce behaviours of concern and provide support that takes the least restrictive form. We achieve this through regular needs re-assessment and the development of person-centred strategies that help people lead full lives in the community and reduce the over-medication of people with a learning disability, autism, or dual diagnosis.
Each individual has a Positive Behaviour Support Plan, and we work closely with relatives and support teams, providing hands-on training to ensure they have the skills needed to implement the plans effectively. We support individuals to learn positive alternatives to concerning behavior and to communicate more effectively, enabling them to lead a happier, more fulfilling life.

Supported Living
Supported living is an alternative to residential care and empowers individuals to choose the right amount of support, when and how they need it, so they can lead an enriching and fulfilling life.
We work with individuals to set their levels of care and support and ensure their rights as a tenant are protected.
Our flexible support is tailored to need, and together we create personalised support plans focusing on interests, priorities, and skills.
This might involve help to find employment or a plan to live more independently in the future.Whatever the need, we are here to help.
The type of support an individual receives is decided by them. From help with personal care needs and managing finances, to finding education, work, or leisure activities each individual decides.
